This is in response to your request for the procedure on how you are going to transact your payment through KIOSK2.
Please refer below procedure for you to upload the payment accordingly.
- Login first using your IMG username/password
- Click +Subpay Kaiser
- Fill-out the Policy Details, search the Planholder's name (last name, first name) in AppNo./Pol No. portion
- Input the Installment No. & Premium amount (policy fees/other fees and uploading of reinstatement form is optional)
- Click "NEXT"
- Fill-out the corresponding information (First Name, Last Name, Email)
- Choose your mode of payment
- Once done, it will now lead you to Your Order Summary. (enter your email address, billing address, etc)
- Choose your Bank then click "SELECT"
- Once completed, click the blue link "CLICK HERE TO SEND US THE PROOF OF PAYMENT FOR VERIFICATION"
- Confirm your deposit. (enter the details of your deposit payment)
- Click +Select file to upload the proof of payment
- Click Submit
