If a plan is already marked as lapsed, an application for Plan Reinstatement must be accomplished and submitted with a payment of amount due per computation provided that includes surcharge and reinstatement fee of P300 fot the plan to become active.
if a plan has NOT YET been marked lapsed, payment of all premium due amounts will suffice for plan to become active.
